(CWS) is pleased to be able to provide the Advanced Practice Provider Well-Being Index, Employee Well-Being Index, Medical Student Well-Being Index, Nurse Well-Being Index, Physician Well-Being Index, and Resident/Fellow Well-Being Index for academic and research purposes.
CWS will provide you with the Well-Being Index for free as long as you agree to certain conditions regarding your use. The conditions described are necessary to ensure that CWS’ interests in any possible commercialization are protected and to ensure that the Well-Being Index is used solely for the research reported. Such use shall be for either the completion of the research or one (1) year from the date of this agreement.
